At Daavlin, we make prescribing home phototherapy units for your patients easy!

Our Home Order Forms are available HERE to download and print. This packet can then be given to your patient. You should also provide your patient with a prescription that indicates lamp type ( Narrowband UVB, Broadband UVB, or UVA ) and a Letter of Medical Necessity.

Your patient can easily work together with our Home Sales and Insurance Processing Departments to fill your prescription. Once the unit has shipped, your office will receive a packet including the time charts and exact output of their unit. Your patient will continue to see you for their home phototherapy treatment schedule and follow-up exams!

Please Note: Daavlin does not provide treatment dosages to  patients. We encourage them to seek this advice from you, their doctor. If you would like assistance, please feel free to call us for information.
